Provide Documentation
You are responsible to provide the Office on Disabilities with documentation, test reports, and/or school records relating to your disability and requested accommodations. This confidential information will be revealed ONLY by written consent from the student. This information will help the Office on Disabilities provide reasonable academic accommodations and support services.
Request Accommodations in a Timely Fashion
A three week notice is required to arrange some accommodations. Though we try to honor late requests we cannot guarantee results. Immediate assistance may be limited due to resource availability.
Report to the Office on Disabilities Prior to Each Semester Start
It is the student's responsibility to provide the Office on Disabilities with their course schedule prior to each semester start, thereby allowing time to make arrangements to provide requested accommodations. Make your appointment three weeks prior to semester start to help insure successful outcome.
